    After submitting the form, we'll email you a secure drive where you can upload your fertility panel, a copy of your passport, and the hospital intake document.

  • MEET WITH THE DOCTOR
    Once the documents are ready, we'll send you a payment link for $150 USD, reimbursed when you book your treatment through Bloom within 3 months of the consult and confirm your consultation time.
